    What's included in the base monthly rate vs. billed separately?

    Monthly Rate Inclusions: - 24/7 personal care and supervision - Three meals daily plus two snacks - Bed linens (sheets, pillowcases, blankets) Additional Charges: The following services and items are billed separately, as they vary by resident need: - Special night-shift accommodations — additional staffing or care arrangements required outside of standard overnight supervision (e.g., one-on-one monitoring, frequent repositioning, or extra check-ins due to fall risk, wandering, or medical needs) - Specialty nutritional drinks — supplements such as Ensure or Boost - Incontinence supplies

    How often do rates typically increase, and by how much?

    Rates will not increase except as outlined in this Residency Agreement. Rate increases are tied to a documented change in the resident's condition that results in a higher level of care being required. Care needs are evaluated using a care level assessment conducted at admission, which classifies residents into one of three levels: mild, moderate, or high. If a reassessment — based on staff observation, physician input, or updated care plan documentation — determines that a resident's needs have increased to a higher care level, the monthly rate may increase.

    A $500 hold fee is required to reserve a room for up to two weeks following a confirmed move-in decision. This fee is non-refundable except in involuntary circumstances (e.g., hospitalization, medical emergency) or death.

    How is long-term care insurance billed and processed?

    Reimbursement model: The facility bills the resident/family directly each month. The family pays the facility, then submits a claim with an invoice to the LTC insurer for reimbursement up to the policy's daily/monthly benefit cap.

    How is level of care assessed, and how often is it reassessed?

    Service Plan Creation: Arizona requires a written service plan to be completed no later than 14 calendar days after the resident's date of acceptance. This plan documents the resident's medical or health problems, the level of service expected, and the amount, type, and frequency of assisted living services being provided. Arizona sets two distinct triggers for updating the service plan: change of condition and routine scheduled reviews (supervisory care every twelve months, personal care every six months, and directed care every three months).

    Two service levels (your license type determines which applies): Medication Administration — staff actually administers the medication under policies and procedures reviewed and approved by a medical practitioner, registered nurse, or pharmacist. Assistance with Self-Administration — staff reminds, opens containers, observes, and verifies that the resident taking the medication is the person on the label, is taking the correct dose, and is taking it at the correct time per the label or a practitioner's order.

    Schedule – Activities are offered, not mandatory; residents choose to participate or opt out. Room setup – Residents may personalize their room with their own furniture, photos, and decor (subject to safety guidelines). Visitors – Residents may receive guests at their discretion, with a 10 am–6 pm visiting hours window (unless special considerations were discussed). Meals – Set meal times, with menu/dietary preferences accommodated where possible. Healthcare providers – Residents keep their own physician, pharmacy, and outside providers.

    How are shared rooms and roommate situations handled?

    No more than two individuals may reside in a shared bedroom or residential unit. Each individual in a shared bedroom must have at least 60 square feet of floor space (not counting closet or bathroom). Each sleeping area must have a door, unimpeded access, and access to natural light; shared bathrooms must provide privacy when in use. Facilities typically match roommates based on care needs, gender preference, and compatibility.

    What security measures are in place, especially for memory care units?

    Cameras – Common areas monitored for safety (not in private bedrooms or bathrooms). Door alarms – Exterior doors and memory care unit entries equipped with alarms to prevent unsupervised exits. Secured entry/exit – Memory care units have controlled access points to reduce elopement risk. Whereabouts checks – Staff conduct regular checks on resident location and safety, day and night, with more frequent checks for memory care residents.

    What's the emergency/disaster plan (power outages, fires, medical events)?

    Written disaster plan covering resident relocation, medical record access, medication availability, and food/water during a disaster — reviewed at least once every 12 months. Power outages – Backup lighting/flashlights and battery-operated smoke detectors as required by code. Fires – Fire extinguisher rated at least 2A-10-BC mounted on-site; smoke detectors tested and documented. Drills – Disaster drills conducted quarterly (staff only); evacuation drills every 6 months (staff and residents).     Resident-initiated move-out: Notice period is set by your Residency Agreement. Facility-initiated termination: - 30 days' written notice for general reasons - 14 days' written notice for nonpayment or specific service-related conditions - No notice if the resident poses an immediate health/safety threat Refunds: No refunds.     How is the adjustment period handled for new residents emotionally?

    The adjustment period is handled with care and patience. The staff understand that it can last 2–4 weeks. During this period, staff will stay vigilant in encouraging residents to help prevent exclusion and isolation.
