    Cost & Finances5
What's included in the base monthly rate vs. billed separately?

We offer three meals a day and two snacks. Certified caregivers are on-site 24/7. We do not offer full-time night shifts, but caregivers are available via call button/camera. Continence supplies are not covered by the home (briefs, wipes, and pads).

How often do rates typically increase, and by how much?

No rate increases unless specified during admission.

Is the home approved for Arizona Long Term Care (ALTCS)? If so, which one?
    Yes

UnitedHealthcare Community Plan (Arizona Physicians IPA, Inc.)

What's the deposit/community fee? Is any portion refundable?

We require a nonrefundable $500 deposit for a two-week hold.

How is long-term care insurance billed and processed?

LTC will send documents to the home to sign and submit every month. Families will pay the rent, and LTC will send refunds via mail.

Level of Care & Health Needs5
How is level of care assessed, and how often is it reassessed?

Care is assessed upon admission and with any change of condition.

Can a resident stay if their care needs increase?
    Yes

A move will be required when the scope of care is beyond the home's capabilities.

What's the staff-to-resident ratio during the day and overnight?

    5 residents: 2 staff

    Are nurses on-site 24/7?
    No
How is medication management handled and coordinated with outside doctors?

Medications are stored in a locked area. Certified caregivers will administer medications and notify outside providers or family to request refills.

    Staff & Quality3
What is the staff turnover rate?

The current caregivers have worked in the home for 2-3 years.

Do caregivers receive specialized dementia / memory-care training?
    Yes
Are recent state inspection reports available to families on request?
    Yes
    Daily Life & Fit5
What does a typical day look like — meals, activities, social programming?

The meals are served to meet residents' schedules. A standard timeframe for breakfast is around 7-9, lunch 11-12:30, and dinner is served around 4-5:30. Activities are scheduled either between breakfast and lunch or between lunch and dinner. We encourage socialization among our residents and staff.

Can we try a meal before committing?
    No
How much choice/autonomy do residents have (schedules, room setup, visitors)?

Residents' autonomy depends on their level of care in our home. We encourage personalization of their rooms. We have visitation hours (10 am-6 pm) that are open to residents' family and friends.

    Is the community pet-friendly?
    Yes
How are shared rooms and roommate situations handled?

Shared rooms are available for same-sex residents. We do our best to maintain privacy and comfort when one bedroom is shared between two residents.

    Safety & Environment3
What security measures are in place, especially for memory care units?

The home is equipped with door alarms and coded locks to keep entries and exits monitored.

What's the emergency/disaster plan (power outages, fires, medical events)?

We relocate residents to nearby homes or discuss possible discharge to families' homes if necessary.

Is the layout wheelchair and mobility accessible throughout?
    Yes
    Transition & Exit3
What's the notice period and refund policy if a resident moves out or passes away?

Admission documents will outline move-in and move-out requirements. What remains constant is a 30-day notice before move-out.

How is the adjustment period handled for new residents emotionally?

We understand that adjusting to a new space is difficult for new residents. We offer unique services that respect all residents' dignity and space. We put our best foot forward and understand that patience and understanding are key within the first month of move-in.

Is a trial stay option available?
    Yes

We consider admissions for trial stays of 2-4 weeks.
